• Requirement of Leased Driver:

1. Take physical (charged to settlement initially, refunded after 30 days).
2. Take drug test.
3. Meet all qualifications required of company drivers.

1. Application must be approved.
2. Run under company log program.
3. Must turn in all fuel tickets.
4. Must maintain a company approved MVR rating.

4. Equipment must meet company requirements and be inspected on a specified schedule.

5. Work Comp Insurance

1. Owner who drives - may sign affidavit which exempts himself.
2. Non-owner must be covered by Work Comp Insurance and owner must provide insurance certificate.

6. Bob Tail Coverage is required.

7. Must possess IRP license plate with 80,000 lbs. limit which is good in all states in which they plan to run.
